Once again this year, we ask parents to download and print the annual Health Form, which is due in the Nursing Office by Monday, August 15.
- If your child has a diagnosis of Asthma and/or Allergies;
if your child has a chronic medical condition that requires medication
to be dispensed at school, the respective Action Plan/Medication forms
must be attached to the Health Form.
- If your 5th through 12th grader is participating in the BFS
Athletics Program, the Athletics Supplement must be attached to the
Health Form.
